Please complete this form with a check for $110.00 payable to St. Mary Sports Club and bring to school in an envelope addressed to Jim Herdegen, St. Mary Volleyball. Parental Consent for Participation

I understand that participating in a sport may result in injury. I certify that my child/children has/have no injury that would limit his/her participation. I hereby release, exonerate, and discharge St. Mary School and the parent volunteers from any injuries occurred durning volleyball season. I have medical coverage and will be responsible for any medical charges related to her. I give my child/children permission to participate in the St. Mary Girls Volleyball Program.
