Pizza Days

PIZZA DAYS – Only One Order!

2011-2012 School Year

This will be the only pizza order for the year. If you need to change your order after you have submitted your initial order, please print off a copy of the order form from the St. Mary Website (Forms) and send it to me with payment. Pizza Days are held on the third Wednesday of each month. Pizza Days are as follows:

September 21; October 19; November 16;
December 14; January 18; February 15;
March 14; April 18; and May 16.

You will order for one month on the grid below, figure out the cost, and then multiply by nine. I will be placing the SAME pizza order for each pizza day.

Orders are due no later than Friday, September 16. Please be prompt in returning this form, EVEN IF YOUR CHILD DOES NOT WANT PIZZA. Thank you for your timely response in submitting your child’s/children’s pizza order form. There will be no follow-up phone calls if you forget to send in your form.
